Usually, these events are the result of a user doing something unwise. Modern rechargeable lithium-ion batteries are engineered to anticipate and combat thermal runaway—the feedback loop in which a battery can overheat and explode. The occasional incidents you hear about are when these protections have been compromised (or were never put in place by cut-rate manufactures in the first place).
